Supported Living Consultation

Community Partnerships has been selected to provide Supported Living Consultation Services for Imagine!. Optional Supported Living Consultation includes assistance with decision-making, planning daily activities and direct assistance to access community resources and/or service providers. It's purpose is to coordinate Supported Living Services with other specialized services in the community on a more frequent and specific basis than is possible through Case Management. Our counselors play an advisory role helping people who have a developmental disability learn how to make choices. Supported Living Consultation is intended to provide intermittent assistance with organization, planning and decision-making, not the actual physical assistance for ongoing services such as daily or weekly appointments or regular program activities. Our Supported Living Consultation includes:

• Decision-making with issues that effect Supported Living Services (optional)
• Support and Service Development (optional)
• Monitoring Quality of Services
• Documentation of Services
• Tracking Plan Expenditures
• Consumer Satisfaction Documentation
