Community Partnerships provides mentorship services to adults with cognitive disabilities so they can live with their families and friends or on their own. By developing partnerships with individuals and their families and coordinating resources that foster independent living, personal contribution and dignity of choice, Community Partnerships works toward an integrated community.
Community Partnerships staff members are actively connected with the community and excel in problem solving and helping people navigate the human services system.
Community Partnerships provides mentorship services including:
- Parenting skills
- Connecting consumers with community resources
- Interviewing and choosing new providers
- Assistance with housing issues such as finding apartments, moving arrangements, etc.
- Assistance with medical appointments and communication with health professionals
- Arranging transportation
- Signing consumers up for approved classes
- Money management
Community Partnerships collaborates with other Imagine! departments to provide assistance with:
- Administering funding plans
- Ordering services
- Tracking planned expenditures
- Benefit management
