Case Management serves as the gateway to services at Imagine!. Once individuals qualify for support through Imagine!, they, along with their family, guardians, friends and program staff, meet with a case manager (or children’s service coordinator for children ages birth to 3) to discuss which services can help them reach their goals.
The case manager/service coordinator then drafts an Individual Plan (IP) or, for children ages birth to 3, an Individual Family Support Plan (IFSP). These plans focus on the issues discussed in the meeting and outline the strategy for addressing them. Working directly with individuals and their families, the case manager helps obtain services for the consumer. Frequent evaluations of an IP or IFSP ensure that the strategy remains relevant to each individual’s needs.
Services provided through Imagine! are paid for by various federal, state and county dollars, insurance reimbursements and out-of-pocket payments. Case management staff are familiar with payment options and can give families specific recommendations regarding funding and funding sources.
Imagine!’s case managers work with more than 219 service providers to ensure that consumers and their families have access to a wide and varied range of options when choosing services. |
